- Hinatao Energy will jointly use part of personal information with joint users as set forth below.
Joint use 1
[ Jointly used personal information ]
- Customers’ name, address, telephone number, detail of retail supply contract or electricity supply contract (hereinafter collectively “Retail Supply Contract”), date of birth, usage of services, information related to energy charge and usage of energy etc.
- Usage of energy related equipment at customers’ premises such as gas pipe, gas meters, gas plugs, type and name of gas or electricity equipment, etc.
- Request details and visit history to customer as well as detail and date of sales, repair, etc.
- Information regarding service points (at which electricity is received); details of electricity transmission and distribution utilities, which execute contracts for wheeling services, such as their service areas, targets covered by the general provisions for isolated island services, supply (electricity receipt) point identification numbers, contractual amounts under wheeling service contracts, upper limits of electric current, electricity connecting transmission service menus, power factors, supply methods, methods of decision-making under wheeling service contracts, meter information, lead-in pole numbers, the availability of grid interconnection equipment, dates of changes made in wheeling service contracts, meter reading day, contractual statuses and methods of business abolition.
- Negawatt trading information; power generation sales amount, demand procurement amount, demand suppressed amount and baseline.
- Information concerning service points: supply point identification numbers, meter information, the availability of load measuring instruments, information on gas meter valve locations, information on meter reading, supply pressure, information on changes made in gas transportation service contracts and information on buildings.
- Information on the safety of Gas Appliances, etc. associated with service points: information concerning the notification prescribed in paragraph 4 of Article 159 of the Gas Business Act including “Information on the item common to the notification which was prescribed in Article 159, Paragraph 4 and the investigated item after the acceptance of application for gas use” before the amendment of the Gas Business Act (April 1, 2017).

- Basic information: customers’ name, address, telephone number, and contract number for their Retail Supply Contract).
- Information regarding service points (at which electricity is received): details of electricity transmission and distribution utilities, which execute contracts for wheeling services or power generation quantity adjustment services (hereinafter collectively, “Contracts for Wheeling Services, etc.”), such as their service areas, targets covered by the general provisions for isolated island services, supply (electricity receipt) point identification numbers, contractual amounts under Contracts for Wheeling Services, upper limits of electric current, electricity connecting transmission service menus, power factors, supply methods, methods of decision-making under Contracts for Wheeling Services, meter information, lead-in pole numbers, the availability of grid interconnection equipment, dates of changes made in Contracts for Wheeling Services, meter reading day, contractual statuses, and methods of business abolition.

- Negawatt trading information: entities which executed relevant negawatt trading contract.
*1: Hinatao Energy jointly use personal information only with business operators whose involvement is necessary to accomplish the purposes of joint use, which does not necessarily mean that we jointly use such personal information among all electricity retailers and electricity transmission and distribution utilities.
*2: Electricity retailers are business operators to which none of the grounds for refusal of registration under paragraph 1 of Article 2-5 of the Electricity Business Act (Act No. 170 of July 11, 1964) applies, and which are registered as electricity retailers by the Minister of Economy, Trade and Industry. *4: A negawatt trading contractor is a business operator who has endered into a demand restraint amount adjustment supply contract with an electricity transmission and distribution utility (including those who has acquired a business operator code before entering into the contract). (For the name, location, etc. of the business operator, please refer to the website of the OCCTO).
*5: Intermediary service to terminate Contracts for Retail Supply, etc. means that a business operator that has received a request for the execution of a new Contract for Retail Supply, etc. from a customer implements procedures through switching support system for terminating the existing Contract for Retail Supply, etc. between the customer and the current business operator on behalf of the customer.

- Information on the safety of Gas Appliances, etc. associated with service points: information concerning the notification prescribed in paragraph 4 of Article 159 of the Gas Business Act including “Information on the item common to the notification which was prescribed in Article 159, Paragraph 4 and the investigated item after the acceptance of application for gas use” before the amendment of the Gas Business Act (April 1, 2017). - Information concerning service points: general gas pipeline service providers whose service areas cover relevant service points (including service points associated with the specified gas pipeline service business conducted by general gas pipeline service providers).
- Information concerning the safety of gas consumption equipment, etc. associated with service points: gas retailers that have executed Retail Supply Contract (also, in relation to information on consumers receiving last resort services, the general gas pipeline service providers whose service areas cover the relevant service points).
*1: We jointly use personal information, for each item of information, only with business operators whose involvement is necessary to accomplish the purposes of joint use, which does not necessarily mean that we jointly use such personal information among all gas retailers and the general gas pipeline service provider.
*2: The Gas Business Act means the Gas Business Act (Act No. 51 of March 31, 1954) amended pursuant to Article 5 of the Supplementary Provisions of the Act for Partial Revision, etc. of the Electricity Business Act, etc. (Act No. 47 of June 24, 2015).
*3: Gas retailers are business operators to which none of the grounds for refusal of registration under paragraph 1 of Article 6 of the Gas Business Act (*2) applies, and which are registered as gas retailers by the Minister of Economy, Trade and Industry (including business operators deemed to be registered as gas retailers pursuant to the Supplementary Provisions of the Act for Partial Revision, etc. of the Electricity Business Act, etc. [Act No. 47 of June 24, 2015]). *4: General gas pipeline service providers are business operators that have obtained a license pursuant to Article 35 of the Gas Business Act (*2) (including business operators deemed to have obtained a license to operate as a general gas pipeline service provider pursuant to the Supplementary Provisions of the Act for Partial Revision, etc. of the Electricity Business Act, etc. [Act No. 47 of June 24, 2015]). *5: Intermediary service to terminate Retail Supply Contract means that a business operator that has received a request for the execution of a new Retail Supply Contract from a customer implements procedures for terminating the existing Retail Supply Contract between the customer and the current business operator on behalf of the customer.
*6: Pursuant to the provision of paragraph 4 of Article 159 of the Gas Business Act,(*2) gas retailers notify gas appliance investigation results to general gas pipeline service providers that engage in transportation services of gas used for gas retail businesses and the relevant general gas pipeline service providers will receive such investigation results.
Disclosure and corrections, etc
- When a customer requests to disclose or correct their own Customer Information held by Hinatao Energy, we will confirm that the person making the request is in fact the customer concerned and then fulfill the request within a reasonable time period and to a reasonable extent under relevant laws, regulations and guidance.Please fill in the required items on our designated application form and send it with documents that can prove your identity.
- In the case of disclosure of Customer Information, the prescribed fee and mailing fee (actual cost incurred) will be invoiced.

- Disclosure, corrections, suspension of use and deletion procedure of Customer Information
When a customer requests to disclose, correct, suspension of use and deletion of their own Customer Information held by Hinatao Energy by procedure set forth below, we will confirm that the person making the request is in fact the customer concerned and then fulfill the request within a reasonable time period and to a reasonable or operationally necessary extent pursuant to applicable laws and regulations.

- In principle, the person themselves. However, an agent delegated by the person, a legal agent of a minor or an adult ward may request disclosure, corrections, suspension of use and deletion of Customer Information on behalf of the person.

- Please fill in the required items on our designated application form and send it to our Contact Counter below along with the required identity verification documents. If you choose to make request by e-mail, please send e-mail with the same documents attached.
- For more details of the request procedure including types of applicable identity verification documents and precautions, our response to disclosure request, cased we cannot accept disclosure request and payment method of fees invoiced, we will explain at the time the customer makes a request.

■ 個人情報保護方針
株式会社ヒナタオエナジー（以下「弊社」といいます）は、個人情報を適切に保護し正しく取り扱うことが事業活動の基本であり、重要な社会的責務と考えております。その責務を果たすため、弊社は、個人情報保護方針を以下のとおり定め、個人情報の保護に最大限努力いたします。
法令の遵守
弊社は、個人情報の保護に関する法律その他の関係法令・ガイドライン等を遵守するとともに、本個人情報保護方針および社内規程を整備し、継続的にその改善に努めます。
取得・利用
弊社は、業務を適切かつ円滑に遂行するため、個人情報を適正な手段により取得いたします。取得にあたり、ご本人に利用目的をあらかじめお知らせするとともに、利用目的の達成に必要な範囲内で利用いたします。
情報の管理
弊社は、個人情報の漏えい、滅失または毀損の防止その他の個人情報の安全管理のために必要かつ適切な措置を講じます。弊社が講じる安全管理措置には、次に掲げる事項が含まれます。
●基本方針の策定
・個人情報の適正な取扱いの確保のため、「関係法令・ガイドライン等の遵守」、「安全管理措置に関する事項」等について定める本個人情報保護方針を策定しています。
●個人情報の取扱いに係る規律の整備
・個人情報の取扱方法、個人情報保護の責任者・担当者およびその任務等について定める個人情報の取扱規程を策定しています。
●組織的安全管理措置
・個人情報の取扱いに関する責任者を設置するとともに、個人情報を取り扱う従業者および当該従業者が取り扱う個人情報の範囲を明確化し、個人情報の保護に関する法律や個人情報の取扱規程に違反している事実または兆候を把握した場合の責任者への報告連絡体制を整備しています。
・個人情報の取扱状況について、定期的に自己点検を実施するとともに、監査部門による監査を実施しています。
●人的安全管理措置
・個人情報の取扱いに関する留意事項について、従業者に定期的な研修を実施しています。
・個人情報についての秘密保持に関する事項を就業規則に記載しています。
●物理的安全管理措置
・個人情報を取り扱う区域において、従業者の入退室管理および持ち込む機器等の制限を行うとともに、権限を有しない者による個人情報の閲覧を防止する措置を実施しています。
・個人情報を取り扱う機器、電子媒体および書類等の盗難または紛失等を防止するための措置を講じるとともに、事業所内の移動を含め、当該機器、電子媒体等を持ち運ぶ場合、容易に個人情報が判明しないよう措置を実施しています。
●技術的安全管理措置
・アクセス制御を実施して、担当者および取り扱う個人情報データベース等の範囲を限定しています。
・個人情報を取り扱う情報システムを外部からの不正アクセスまたは不正ソフトウェアから保護する仕組みを導入しています。
●外的環境の把握
・個人情報を保管している外国における個人情報の保護に関する制度を把握した上で安全管理措置を実施しています。
第三者への提供
弊社は、法令・ガイドライン等により第三者への提供が認められている場合および委託・共同利用など第三者に該当しないとされている場合を除き、ご本人の同意を得ることなく、個人情報を第三者に提供いたしません。また、委託先等に提供する場合には、個人情報の管理に関して必要な水準を満たす者を選定し、個人情報保護に関する取り決めを行うとともに適切に監督いたします。
開示・訂正等
ご本人が、個人情報の開示・訂正等を希望される場合、弊社は、ご本人であることを確認させていただいた上で、法令・ガイドライン等に基づく合理的な範囲において、速やかに対応いたします。

弊社においては、お客さまの個人情報を、ガス・電気・熱等のエネルギー供給販売業（エネルギーの調達を含む）、住宅設備機器・機械器具の小売業、設備工事業、土木建築工事業、建築リフォーム業、警備防災業、総合リース業、金融・保険業、生活関連サービス業、クレジットカード業、不動産賃貸・管理業、教育支援業およびこれらに附帯する事業、ならびに関連するアフターサービスの提供および上記各種事業に関するお知らせのために利用いたします。
また、取得した閲覧履歴や購買履歴等の情報を分析して、弊社サイトのご利用や弊社の新しいサービスに関するアンケートの送付、及び新しい商品やサービスの開発にために利用いたします。

お客さま情報と委託・受託業務
弊社は、各種事業を円滑に遂行するため、金融機関、情報処理会社、関係会社、協力企業（工事会社等）等に業務の一部を委託することがあります。その場合、提供した個人情報が受託の目的に限り、安全かつ適切に利用されるよう委託先を監督いたします。
また、弊社は、業務委託を受け、それに伴い第三者から個人情報を取得する場合がございます。この場合、当該個人情報は、委託された事業の目的の範囲内でのみ使用いたします。なお、個人情報の利用にあたっては、公正競争の確保に十分配慮いたします。

※2 小売電気事業者とは、電気事業法（昭和39年7月11日法律第170号）第2条の5第1項に規定する登録拒否事由に該当せず、資源エネルギー庁のホームページに掲載されている登録小売電気事業者一覧記載の事業者をいいます。事業者の名称、住所および代表者氏名については、同ホームページをご参照ください。
※3 一般送配電事業者とは、北海道電力株式会社、東北電力株式会社、東京電力パワーグリッド株式会社、中部電力株式会社、北陸電力株式会社、関西電力株式会社、中国電力株式会社、四国電力株式会社、九州電力株式会社及び沖縄電力株式会社をいいます。
※4 需要抑制契約者とは、一般送配電事業者たる会員との間で需要抑制量調整供給契約を締結している事業者（契約締結前に事業者コードを取得している事業者を含みます）をいいます（事業者の名称、所在地等については、電力広域的運営推進機関のホームページをご参照ください）。また、代表者氏名については、各社のホームページをご参照ください。
※5 「小売供給等契約の廃止取次」とは、お客さまから新たに小売供給等契約の申込みを受けた小売電気事業者が、スイッチング支援システムを通じて、お客さまを代行して、既存の小売電気事業者に対して、小売供給等契約の解約の申込みを行うことをいいます。

供給地点に関する情報：供給地点を供給区域とする一般ガス導管事業者（一般ガス導管事業者が行う特定ガス導管事業の供給地点を含む。）
供給地点に関する消費機器等の保安に関する情報：小売供給契約を締結しているガス小売事業者（但し、最終保障供給を受けている需要者に関する基本情報については、供給地点を供給区域とする一般ガス導管事業者）
※1 弊社は、共同利用の目的のために情報項目ごとに必要な範囲の事業者を限定してお客さまの個人情報を共同利用するものであり、必ずしも全てのガス小売事業者および一般ガス導管事業者との間でお客さまの個人情報を共同利用するものではありません。
※2 ガス事業法とは、電気事業法等の一部を改正する等の法律（平成27年6月24日法律第47号）第5条による改正後のガス事業法（昭和29年3月31日法律第51号）をいいます。
※3 ガス小売事業者とは、ガス事業法（※2）第6条第1項に規定する登録拒否事由に該当せず、ガス小売事業者として経済産業大臣の登録を受けた事業者（電気事業法等の一部を改正する等の法律（平成27年6月24日法律第47号）の附則により、ガス小売事業者の登録を受けたとみなされた事業者を含みます。）をいいます（事業者の名称、所在地等につきましては、資源エネルギー庁のホームページをご参照ください）。
※4 一般ガス導管事業者とは、ガス事業法（※2）第35条の許可を受けた事業者（電気事業法等の一部を改正する等の法律（平成27年6月24日法律第47号）の附則により、一般ガス導管事業者の許可を受けたとみなされた事業者を含みます。）をいいます（事業者の名称、所在地等につきましては、日本ガス協会のホームページをご参照ください）。
※5 「小売供給契約の廃止取次」とは、お客さまから新たに小売供給契約の申込みを受けた事業者が、お客さまを代行して、既存の事業者に対して、小売供給契約の解約の申込みを行うことをいいます。
※6 ガス事業法第159条第4項の規定により、ガス小売事業者は、そのガス小売事業の用に供するためのガスに係る託送供給を行う一般ガス導管事業者に対し、消費機器調査の結果を通知し、一般ガス導管事業者はその結果を受領します。

■ お客さま情報の開示、訂正、利用停止等の手続き
●個人情報の開示について
弊社は、以下の手続きによる開示請求があった場合には、法令に基づく合理的な範囲において、個人情報を開示します。
開示請求ができる方
原則として、ご本人です。ただし、ご本人が委任した代理人、未成年者または成年被後見人の法定代理人は、ご本人に代わって個人情報の開示請求をすることができます。
開示請求の方法
当社所定の「お客さま情報開示申込書」に必要事項をご記入のうえ、次の書類を同封して、下記の宛先まで郵送してください
1.開示対象者ご本人が申請される場合

なお、開示にあたっては、ご本人であることを証明する書類が必要となりますので、申込書記載内容をご確認の上、ご準備、ご申請ください。
2.開示対象者の代理人の方が申請される場合

なお、開示にあたっては、
・開示対象者が逝去された日付と開示場所に居住されていたことを証明する書類
・代理人がご本人であることを証明する書類
が必要となりますので、申込書記載内容をご確認の上、ご準備、ご申請ください。
なお、代理人資格を証明する書類は下記のとおりとなりますので、該当する書類１点を準備してください。

運転免許証：住所等が変更されている場合は、裏面の写しも必要です。
印鑑登録証明書：（原本）を同封してください。
住民票の写し：発行日より6か月以内の原本を同封してください。
健康保険証：住所が記載されていない場合や住所が変更されている場合は、現住所をご記入の上、その写しを同封してください。
パスポート：写真および住所のページの写しを同封してください。

3. 開示請求に必要な費用
(1) お支払いただく金額
お客さま情報の開示請求をいただく場合には、書面交付の手数料および郵送料等として、1件につき1,000円（3項目まで。4項目以降1項目につき100円加算）をお支払いいただきます。
(2) お支払い方法
開示請求の結果（回答書）を配達した郵便局員にお支払いください（代金引換郵便）。
4. 開示請求に対する弊社の対応について
(1) 回答方法
弊社で発行した回答書は郵送（代金引換郵便）でのみ発送いたします。
(2) 回答までの期間
弊社は、開示申込書を受領した日から原則として10営業日（土・日・祝日除く）以内にお客さま情報を開示するかどうかを決定し、その旨をお知らせいたします。ただし、開示請求の内容確認、事務処理上の事情及びその他正当な理由により、その期間を延長する場合がございます。
(3) 開示できない場合
弊社は、次のいずれかに該当する場合には、個人情報の全部または一部を開示しないことがあります。その場合は理由をお知らせするように努めます。
申込書の記入内容、同封書類に不備があった場合
ご本人（開示の対象となる方・代理人）であることが確認できない場合
代理人からの申込みで、代理人資格が確認できない場合
開示請求項目が個人情報保護法に定める「保有個人データ」に該当しない場合
開示請求項目の特定に足りる事項をご提示いただけない場合
所定の開示手数料等をお支払いいただけない場合
ご本人または第三者の生命、身体、財産その他の権利利益を害するおそれがある場合
弊社の業務運営に著しい支障を及ぼすおそれがある場合
他の法令等に違反する、もしくは恐れがある場合
●個人情報の訂正、追加、削除について
個人情報の内容が事実でない場合に限り、弊社はお客さまのお申し出に応じ、業務運営上必要な範囲内において、個人情報の訂正、追加（以下「訂正等」といいます。）を行います。
訂正等の請求ができる方
原則として、ご本人です。ただし、ご本人が委任した代理人、未成年者または成年被後見人の法定代理人は、ご本人に代わって個人情報の利用停止等の請求をすることができます。
訂正等の請求の窓口・方法について
訂正等の内容により、受け付け可能かを確認いたしますので、弊社窓口までご連絡の上、ご確認ください。
● 個人情報の利用停止、消去、第三者提供の停止について
弊社は法令に従い、下記の場合に該当する場合には、お客さまのお申し出に応じ、法令等に定める範囲内において、個人情報の利用停止、消去を行います。
(1)弊社が定める利用目的以外で利用した場合
(2)弊社が法令に照らし、不適正な利用を行った場合
(3)弊社が法令に照らし、不正な手段で取得した場合
(4)弊社が定める利用目的に照らし、利用する必要がなくなった場合
(5)法令に規定する漏えい、滅失、毀損等が生じた場合
(6)法令に照らし、お客さまの権利又は正当な利益が害されるおそれがあると判断できる場合
また、弊社が個人情報を法令に違反して第三者に提供している場合、または上記（４）～（６）の場合には、弊社はお客さまのお申し出に応じ、法令等に定める範囲内において、第三者提供の停止を行います。
なお、利用停止・消去・第三者提供の停止（以下「利用停止等」といいます。）を行った場合および行わない場合、いずれにおいてもその旨をお知らせいたします。
利用停止等の請求ができる方
原則として、ご本人です。ただし、ご本人が委任した代理人、未成年者または成年被後見人の法定代理人は、ご本人に代わって個人情報の利用停止等の請求をすることができます。
